I show that Near Ghoul Bezerkers (EG/Often) and
Tasaba Lizardman Shaman (FoS/Rare) both drop it...

Venom is correct. Back when I got mine, it dropped only off the Eye of Beleth; a multi-hour respawn party mob. You have it much easier!

Just spent lvl 22-25 on nothing but neer goul bezerkers at EG area. Still no drop. :confused:
