As far as i know, they are closed when the server comes up after a downtime. After that it depends on if the last player to go through closed the door after passing.

Hmmm.. To quote the movie, Patton; "You know General, sometimes they don't know when you are joking."
The tunnel from Dwarf land to Schutgart is 'open,' and has no door that I could see. It is in a hard-to-find canyon tho. It is just south of Miner Maron (he is to the south of the Strip Mine.)
From orclands it is very easy as well. Just walk to the valley of the lords and voila. Don't try it when you are a lowbie though those Kasha bears are agro and eat lowbies.
Another easy way to reach Elmore is by boat. Get into the boat in Gludin and travel to Rune. Or if you prefer, jump into the water in Dark Elf village (bring a soe) and swim to Rune.
Ps. You can also swim from Rune to Orcland and back (you need to bring a few healign potions though and haste doesnt hurt either).
