I've searched the forums, but couldn't find trace of this complaint.
I've only done two Kartia 90 solos so far, and each time, Stage 4 in the final room was skipped, resulting in the Watcher being spawned about two minutes after the boss. Given an entire stage is skipped, I am confident this mechanic is not by design, but what I am wondering is if this is a 100% occurrence or if most runs are in fact fine.
I've had a similar issue in K85 on occasion where stage 4 would be skipped, but that just resulted in the Watcher never being spawned. It's really, really tough to battle both the boss and a mini-boss at the same time. The run I just did finished with only half of my group mates still remaining.
I do lvl 90 solo Kartia twice a day, and lvl 85 solo Kartia 2-3 times a day, so I'm pretty experienced with it. My observations are the following:
In lvl 85 solo Kartia, if you manage to finish off Dimensional Watcher in the 2nd room (not letting it escape when its hp goes under 50%), the Watcher will not appear at the 4th stage in 3rd room, and the stages will go fast one after another.
If the Watcher escapes from the 2nd room, it will re-appear in the 3rd room at the 4th stage which will then last longer than usual.
In Kartia 90, however, the Watcher normally does NOT appear at the 4th stage, while the long pause between 4th and 5th stages is still there. The Watcher then reappears in the end of stage 7th, while you're battling the final boss. It won't harm you at all, since it just stays in the remote corner, so you can happily finish off the final boss and leave the dungeon.
Only in case if you're stupidly running all over the place you can eventually aggro the Watcher, then you'll have to deal both with the final boss (Pelline) and the watcher at the same time (since you called for it). :p
I do hope this is by design, and NCsoft will not fix this "oddity", because having to fight the Watcher at the 4th stage (like we did in Kartia 85) would really slow things down.
P.S. Pro tip: if you don't want your npc to die, ALWAYS send away the archer since he's still the most bugged npc among them all. Every time I took him instead of the summoner, at least one or two npc died, sometimes as early as in the 1st room.

I think the long delay in the 4th stage is because the Dimensional Watcher should appear there, and if killed fast, 5th stage rolls immediately. But, since watcher appears at last stage, that delay cannot be avoided. Something similar happened a couple months, Dimensional Watcher appeared at 4th stage (K85), but despawned right away, and therefore, delaying the whole stage altogether.
I, being a dwarf dagger, learned to always ditch either the warrior or archer, and i've found that the summoner helps a lot (with his "cage-ing" skill and such), but for healers and ISS, having a summoner there is counterproductive, since heals spread out too long (counting 3 more npcs "to-heal").

Odd, on my Feoh sub I always send away the warrior, and none of my npc's have ever died. In the second room, I find that if I do anythign at all (nuke/debuff/dot) to the miniboss, it chases me around until it gets to half hp and leaves. For that reason I tend to ditch the warrior, that would just be running and trying to get close to the boss, as oppose to the archer that can shoot it from anywhere.
I keep the summoner since its pets do a hate every now and then and seem to move faster than the warrior as well.
The tank and healer are kept for my survival, mostly for the poison stage on first room though.
So, to sum up: Having archer for me is WIN, but I guess having summoner is also WIN, since I have both, and have not died in the instance since hitting 86 on my feoh sub (died once at 85, first time I tried it, unlucky fear >.< )
Most of the times that ****** spawns for me, but from time to time he doesnt.If he spawns its after Pelline and he is not aggresive, stays in one of the corners of the room doing nothing.
Not that I rly care about him, doesnt ruin the Kartia as a whole, but some ppl may be QQing about it as it seems.
So to clear it up, what makes him spawn after pelline.... is if you kill him before he runs out of the room in the second stage?
Hmm I have killed him in 2 nd room 1-nce and he is not re-appearing *.
Not that I killed him as it was mine plan but for some reason he was not saying " I'm leaving" even having 40% HP ...
When he had around around 30+ % he decide to leave but then tank get successful aggro ... when he wa around 15 % I just decided to not him leave :D
And no, he haven't come back.
1st time when I did Kartia 90 I haven't notice watcher at RB room :)
I thought that it is bugged and not come in 4th :D
It was epic fight - I run out of MP, run of buffs and warrior has died
But ultimately I won .. and now I'm happy that I can avoid watcher in RB room :p

And then of course you do not get her during the end boss fight.

I have a feeling I've seen him in game today, I've just guessed it: there was that made-in-April newcomer, standing in Gludio for hours spamming the chat with: "buffs pliz! buffs pliz! buffs pliz! buffs pliz!"

run in hit it run back to first room soon as he moves into first room start dpsin it like a boss
if he falls below 50% hp he will run but he cant run cus door closed so hes stuck and u can finish him off with all the time of the world that was simple..
thus doesnt work that good for melee's
tyrrs have to rush deal damage get ported back to second room /target dim > rush > damage > ported :p (npc's will hit the boss in mean time so u can wait for em to kill it also but i just keep using rush/shadow step on my 2 alts)
same with othels have to do shadow step
tho if u dont slack on othel u can simply keep a poison thing on the floor (slows down mobs) and when he starts to run use throwin dagger u can easily finish him off now atleast i never failed
for the onces who didnt get it
once the door closes and the boss runs back cus his hp felt below 50% u cant GET NEAR THAT DOOR or u get ported into second room so if possible stay away from the door till its dead, when its dead u obviously go near the door and simply do cake wall to 3rd (boss room) room and u will never see a mini boss in last room again thats how i do it but im afraid its going to be "fixed" now :p
but it works good on anything that has ranged on summoner i switch to ranged summons just to kill it
My girlfriend's Healer has been bugged out the last 2 times in solo kartia.
The 2nd room is cleared and she goes into the RB room, and the only NPCs that follow her are the summoner's balls. The other NPCs never spawn, even if she runs deep into the room, or kills mobs, or it goes to stage 2, or if she dies, etc.
Theres never been a problem as severe as this before, but today now it happened twice the exact same way.
what you guys do in kartia is something only god knows
so many people complaining about npcs dieing npcs dont appear or w/e
yesterday i tried to kill warrior npc i failed horrible cus healer pumps a massive heal at 50% hp
so i went to tank same problem
so i went on the healer it self same problem
so what u guys do with these npcs in order to get em to die takes ALOT

Tbh i dunno either, so far my only issue is when healer pops tree isntead of healing when HP is low. but for some months now my kartias been pretty smooth with archer,rogue,tank,healer,summoner and iss.

Yup, same here. No real problems with NPC disappearing etc. It works fine. Only little glitch every now and then is that when I enter room 3, the summoner's summons do not immediately port there together with the rest of the NPC - sometimes they only appear after I kill the first few mobs in stage 1. But that is all, certainly not as horribly bugged as some report in this thread here. The only thing which really annoys me is the "intelligence" of the NPC, like the summoner using exile (or whatever that banishing skill is called) on the miniboss, right after I popped rhapsody >.<
