Item Drop Penalty
The item destruction penalty from the days of yore returns: when a Chaotic character with negative Reputation and a PK count of 4 or higher dies, an item destruction penalty applies even to augmented items. (A character with normal Reputation is not affected by the penalty.) The change of item destruction, and the number of items to be destroy, increases according to the character’s PK count, to a maximum of five items destroyed upon a single death.
Even since Glory days (linked by Light616, the post above yours), the pk count of 4 will make you drop. 3 and below you are fine, but red with 4 pks will risk gear drop.
But seriously? Someone that is willing to go red at +1, will be the same person that wouldnt care going over +4 really.
Sometimes you just go frenzy and shout out '***** it' and just vanish everything on your sight - and I surely wouldnt be the one to reminder such person of that stupid death penalty.

Do you not lose gear when you go red now? I just killed some Red guy and he didnt drop anything. I was like wtf?

You lose and you can't imagine how high change it has to lose.Even when you reach 4 from 3 pk count,run away and exit game immediately,otherwise you will make a nub rich whom you wanted to rampage.It has become my 16th and i keep barely saving my ... in last second.I have dropped too many junks till now,but never experienced destroy on normal items.Destroy applies on mentor and bound gears i guess.
A suggestion if you want to rampage nubs so much : Get timed gear from ncstore (1 Ncoin) they never drop or be destroyed.
